To support with track and trace, the Department for Education have asked schools to assist in identifying close contacts and advise about self-isolation up to Christmas Eve, December 24th. For those pupils developing symptoms within 48hours of last being in school and then subsequently test positive for coronavirus, or if they don't have symptoms undertake a coronavirus test within 2 days of their last day at school, then you must contact school using this form. We will then, as a school, take advice from Public Health England in terms of notifying other children, families, school staff or bubbles to self-isolate. 

If your child needs to isolate during the first week back in January, please contact school using the same form.

To get a test use the NHS COVID-19 app, www.gov.uk/get-coronavirus-test or call NHS on 119. Many test results are now being received within 48 hours of the test.
